Anesthesia was advised when the peritoneum was about to be opened in order to compensate for loss of abdominal pressure. Bile-stained (serum bilirubin was 9) peritoneal fluid was present on opening.

The ascitic fluid was suctioned. No obvious bowel contents were found.

The small bowel was slowly and carefully eviscerated. The entire length of the small bowel was mottled gray and terminal ileum was black.
