A 62-year-old woman with hypercryoglobinemia was given an anticoagulant injection in the left lower quadrant of the abdomen. She developed progressive abdominal pain associated with an expanding subcutaneous left sided abdominal mass.

Abdominal CT scan showed a large fluid collection in the subcutaneous tissue with a density consistent with blood. The collection extended from the lower portion of the right rectus muscle.

The blood supply to the rectus muscle comes from the inferior and superior epigastric vessels. The vessels branch and communicate via multiple small channels in the watershed area around the umbilicus.
