The abdomen was opened through a chevron incision, anticipating the exposure necessary for a radical resection if indicated.

The antral tumor was palpated and found to be fixed posteriorly. Celiac and hepatic portal adenopathy was palpable.

Palpation of the liver revealed multiple hard nodules. One nodule was biopsied and frozen section revealed metastatic adenocarcinoma. The decision was made to attempt palliative resection to alleviate outlet obstruction and eliminate blood loss from the raw surface of the tumor.
