When the sigmoid colon is long and redundant and the near and far ends of the loop are close together, the sigmoid has the potential to volvulize, resulting in a closed loop obstruction. Colonoscopic or hydrostatic reduction may be successful, but the patient may experience repeated episodes and may require resection. On X-ray, the dilated loop has a characteristic lower left to upper right axis.
