An 82-year-old man presented with three episodes of GI bleeding which could not be localized on the first two episodes, which stopped spontaneously. At the third bleed a capsule camera documented a bleeding lesion in the distal duodenum or proximal jejunum. Subsequent upper endoscopy localized the lesion in what was thought to be 4th portion of duodenum. The first Endoscopic view of the lesion is shown.

The lesion occupied half the circumference of a plica semicircularis.

Bleeding was seen from the friable surface. Biopsy showed villous adenoma with high-grade dysplasia. The bowel wall was marked with ink injection proximal and distal to the lesion.
