This appendix had liquefaction necrosis at its base and the base of the cecum was inflamed and indurated. Attempts to close such a cecum are at high risk of breakdown and cecal fistula. The safest approach in such a situation is iliocecal resection as was done in this case. The resection was carried out without difficulty through the McBurney incision.
