A 23-year-old man presented with urinary urgency and frequency associated with vague abdominal pain. Abdominal exam was unremarkable except for some mild suprapubic tenderness. He had had several episodes of non-specific abdominal pain in the past and the workup had been negative. A CT scan showed an enlarged, thickened appendix. The white arrow indicates the base and the yellow arrow the body of the appendix.

The appendix arched upward behind the terminal ileum.

The tip of the appendix descended behind the bladder and lay against the posterior bladder wall.
