A 34-year-old woman presented with a feeling of lower abdominal fullness. Pelvic exam was unremarkable. CT scan showed a large, smooth mass of homogeneous density with internal septations.
A cut at the level of the uterine fundus is shown. The right ovary with a small cyst is visualized to adjacent to the uterus, but the left ovary is not visualized.

The patient was taken to the operating room and the abdomen was opened through a low midline incision.
