An 82-year-old man presented with abdominal pain, nausea and vomiting. His abdomen was distended with generalized but not focal tenderness. Abdominal films including the left lateral decubitus shown demonstrated a dilated colon with air down to the distal descending colon. He had a tender 4-cm bulge below the left inguinal ligament medial to the femoral vessels.

CT scan, obtained in the emergency room prior to surgical consult, shows the markedly dilated colon at the mid-abdominal level.

At the upper end of the femoral canal the sigmoid can be seen entering the femoral space.
