A 56-year-old man presented with a fifth recurrence of a herniation around an ilial conduit stoma site. The abdomen was opened through a midline incision and the stoma site visualized. Two non-absorbable braided 2-0 sutures (shown) were placed to begin to narrow the fascial defect.

The neck of the ilial loop was dissected circumferentially at the fascial level and encircled with a Penrose drain.

The fascial defect was bridged with two 4x7 cm patches of acellular human tissue matrix (see discussion). The matrix was tacked to the transversalis fascia with 2-0 polypropylene. The strips were sutured together, leaving a snug ring around the ilial loop.
