A 30-year-old man presented with a feeling of left buttock fullness on sitting. There were no systemic symptoms. Rectal exam revealed a left-sided mass just above the sphincters. Rectal mucosa was normal on proctoscopic examination.

This axial cut shows the lesion filling the true pelvis. Core needle biopsy showed features consistent with angiomyxoma.
